Project No. 20041046
Dear Mrs. Welch:
At the request of Drahota Construction, we reviewed Terracon observation reports of the
foundation system for the above referenced project. Monitoring by Terracon was provided at
the direction of Drahota Construction on an "as requested" basis. Terracon services generally
consisted of observation of foundation footing excavations "open -hole", reinforcing steel, post -
tension placement/stressing, as well as concrete testing. Our foundation observation services
on the project building utilized the project specifications and the Geotechnical Engineering
Report recommendations (Project No. 20025026) performed by Terracon (dated March 5,
2002).
Reinforcina Steel and Post -Tension
Terracon provided reinforcing steel and post -tension placement observation in structural
concrete placements as requested. In addition, stress tensioning verification was provided with
a copy of all reports provided to the client, general contractor and structural engineer of record
for the project. The observations were performed per the provided plans and specifications for
the project and have been formally reported to the client and designated parties. It is
recommended that the project structural engineer review the test reports for acceptance as
meeting the project design criteria.
Foundation
Terracon provided observation of the post -tension beam footing foundation(s) excavations as
requested during construction. A summary of the observations and structural review response
items were provided in written reports upon completion. Per Terracon's geotechnical report and
observations performed, footing foundations have been placed in general accordance with the
plans and specifications and were placed on the anticipated bearing material identified in our
geotechnical report.

Concrete
The Portland cement concrete placed was tested for field physical properties and laboratory
compressive strength. The reports indicate the specified minimum compressive strength
requirements for the project have been achieved. It is recommended that the project structural
engineer review the test reports for acceptance as meeting the project design criteria.
This report has been prepared for the exclusive use of providing test data and observations to
document the foundation operations and concrete testing associated with the foundation
system (post -tension) operations conducted for the proposed project. The opinions and
conclusions presented have been derived in accordance with generally accepted geotechnical
and material engineering practice. No other warranty expressed or implied is made. In the
event that there are any changes in the nature or design of the project, the conclusions
contained in this report shall not be considered valid unless this firm reviews such changes, and
the conclusions of this report are modified or verified in writing.
This letter is prepared for the exclusive use and reliance of our client. Use or reliance by any
other party is prohibited without the written authorization of our client and Terracon.
It has been our pleasure to work with you on this project and to continue providing services
