Gentlemen:
On May 23, 2007 this office observed and approved the horizontal and vertical reinforcement
placed in the lower portion of the foundation trenches for buildings A, B, C, and D at the
location referenced above. The reinforcement observed meets the specifications for size, type
and installation in accordance with the foundation design drawings by Axiom Structural
Engineering, LLC, project number: MILL-061-107-05-25A1, dated April 25, 2007.

Gentlemen:
On June 27, 2007 this office observed the placement of the tendons and the conventional
deformed type reinforcement in the upper portion of the foundation and slab for buildings A and
C. The reinforcement observed meets the specifications for size, type and installation in
accordance with the foundation design drawings by Axiom Structural Engineering, LLC, project
number: MILL-06I107-05-25A1, dated April 25, 2007.

Gentlemen:
On July 5, 2007 this office observed the elongations of the reinforcing strands. The reinforcing
strands were previously marked at 10 inches from the exterior face of the concrete slab. The
observed elongations are consistent with those indicated on the foundation design drawings by
Axiom Structural Engineering, LLC, project number: MILL-061-107-05-25A1, dated April 25,
2007. Enclosed is a copy of the gage calibration chart for the stressing equipment used to
tension the strands.
