BUILDING INSPECTION Requirements to be Completed By: Za 'l 11/14/2005 TCO FOR 30 DAYS UNTIL UP -FRONT CONTROLS FOR RANGE IN BREAKROOM IS INSTALLED. PER KCH ZONING Requirements to be Completed By: 11/16/2005 ok for tco with the following conditions: install raised HC sign in front of HC spaces, install bike rack, remove construciton equipment, porta potty . Ok for 30 days past issua ce. JLn STORMWATER ENGINEERING Requirements to be Completed By: 11/18/2005 TCO for 90 days. The detention pond outlet box needs to be modified due to the existing outlet pipe being lower than the construction plans indicated. The orifice plate may need to be modified also. Don Park with Park Engineering is working with Mike Simms of Clark Construction.ENGINEERING Requirements to be Completed By: � F �o �l/ ll/ 11/15/2005 TCO for 90 day's, wings on handicap ramps. If such work is not completed within the time set forth above and approved on final inspection, or unless the time for completion is extended by the Building and Zoning Office, an order to vacate the premises, or a violation notice for occupying the building, structure, or land without a Certificate of Occupancy will be issued. Such violation is subject to a maximum fine of $100000..00 per day for every day of violation. Contractor/O ner/Lessee Date Aut orized By Date of Issuance NOTE: The issuance of this temporary certificate of occupancy will entitle the owner to place City Utility service in the owner's name. Construction power rates may be substantially higher than the rate to which the owner is entitled. Until the owner contacts the City Utility Office and makes the necessary arrangements to transfer City Utilities to the owner's name, utilities will continue to be furnished at construction rates and both the owner of the property and the contractor will continue to be liable to the City for all utility bills. 11/18/2005 2:53:06 PM
