THIS AGREEMENT is made and entered into this

after designated as "Owner", and THE CITY OF FORT COLLINS,
COLORADO, a municipal corporation, hereinafter designated as.

WITNESSETH:
WHEREAS, 0 -n-er has applied to the City for a sanitary
sewer tap to serve the property hereinbelow described; and
WHEREAS, Owner has further requested that the City
permit payment of the fees for such tap in installments; and
WHEREAS, in accordance with the resolutions previously
adopted by the City Council such payment, in installments, has
been agreed upon.
NOW, THEREFORE, it is agreed as follows:
1. City hereby grants unto Owner a tap on the City's
sanitary sewer utility to serve the property located at
S0 t/ 4,11r1t4 (insert street address) .
2. The amount due,from Owner to the City for such tap
is Five Hundred Fifty Dollars ($550.00), of which $ C
has been paid by Owner, leaving a balance due of $
Such remaining balance shall be payable to the City in equal

commencing
month from the date of this agreement and on the like day of
each month thereafter until the full balance is paid. In the
event of the failure of Owner to make any monthly payment when
due, the City may declare the entire unpaid balance owing by
Owner immediately due and payable and may begin foreclosing
the lien granted below to collect such balance.
3. To secure the payment of the balance owing, the
Owner hereby grants unto the City a lien on the following
described property served by the sewer tap:
This lien may be foreclosed in the same manner as a real estate
mortgage in the event of failure of Owner to make the payments
called for under this agreement.
4. This agreement shall be binding upon the parties
hereto, their heirs, personal representatives, successors and
assigns.
IN WITNESS WHEREOF, the parties hereto have caused this
agreement to be signed the day and year first hereinabove written.
