Re: Associates in Family Medicine Job No. 2875.0 The site was visited with the following observations: 1. First floor decking was in place and had been rewelded using weld washers as specified. 2. At the northwest comer of the southeast stair at the second floor level, the tube column which was split to go through the beam has caused the bottom flange of the beam to buckle up slightly. It is noted that this stub column is not welded to the top of the bottom flange or the bottom of the top flange except by tack weld. It is welded full height in the connection to the web. There is clear 1/8" space. This should be welded for full end bearing top and bottom. 3. The second floor deck has also been welded with welding washers at approximately 30" on center at the side laps and 3 welds per sheet across the ends. None of the X-braces have been attached. One end is bolted, the other is free at this point. Perineter deck welds are approximately 12" to 16" on center. 4. Footing forms have been placed and reinforcing installed in the foundation for the slab on grade portion at the south end of the building. Reinforcing is not yet elevated to its proper cover. 5. The penthouse mechanical roof deck is erected and composite deck has been installed. The deck has been welded to supports at each rib and at approximately 6" on center around the perimeter. Tek screws are placed at each side lap at 4 screws per space. 6. Twelve- inch -diameter hszles have been cut through the deck at two locations. above items were discussed with the Superintendent at the site. RIFF CONSULTANTS - STRUCTURAL ENGINEERS - 11290 W. Re: Associates In Family Medicine Job No. 2875.0 The site was visited with the following notations: 1. Basement wall pour has been completed. A portion ofbasement slab just inside the latest wall pour has not yet been completed. 2. Installation of continuous angle along the north wall is taking place. Expansion bolts have been installed, and Ironworkers were welding to the existing plates and also installing the stiffeners. We observed the weld across the top of the angle to the existing plates to be an estimated 3/16" in lieu of the 1/4" size specified. This was brought to the attention of the Contractor. 3. Other beam bearing corrections to compensate for misplaced embed plates, as noted on our submitted sketch details, are partially completed. Work observed to date appears to be in keeping with the recommended procedures. 4. Two columns on grid line P at grids 5 and 11 had the attachment plates installed on the columns too low. These have been burned off and moved upward. The quality of construction observed was generally good. Colorado 80521-0503 (970)484-0359 Fax (970) 484-0454 Terracon observed the excavation for the upper foundation level at the referenced project on March 22 and 23, 1999. The majority of soils at the upper foundation level were natural sandy lean clay and engineered fill consisting of sand, lean. clay-. The natural scils were -similar to soils -described in our Geotechnical Engineering Report prepared for the project and dated April 16, 1998 (see Project No. 20985052). Preliminary density tests indicated a relatively small area of uncontrolled fill approximately one foot deep extended approximately 20 to 30 feet southward from the northeast comer of the excavation. Subsequent density tests indicated the uncontrolled fill had been properly recompacted. The density test results are being submitted to you under separate cover. Based on our observations, it is recommended the structure be supported by a spread footing foundation system designed in accordance with recommendations in our report. Specifically, the excavation at elevation 86'-4" was observed. The soils at the main foundation level are sandy lean clay over clayey/silty sand with gravel. These soils are similar to soils described in our Geotechnical Engineering Report prepared for the project and dated April 16, 1998 (see Project No. 20985052). Based on our observations, it is recommended the structure be supported by a spread footing foundation system designed in accordance with the recommendations set forth in our report. At the time of the observation, free groundwater was encountered %2 foot below the main basement foundation level. Hence, a dewatering system should be placed around the basement foundation.
