We have visually observed the footing forms placed at the residence under construction at Lot 72,
Shenandoah, (202 Cheops), a subdivision of Fort Collins, Colorado. This observation was performed
on May 28, 1999.
It is our opinion that the form widths are in substantial conformance with the foundation plan drawn
by MNA, Inc., Plan No. 1024/GL. No substantial amounts of loose material were noted below
footing, wall or pad bearing locations.
This observation does not, nor was it intended to cover setback requirements. It is the
contractors/owners responsibility to verify that the home has been located within the appropriate
building setbacks.

W have visually observed the wall reinforcing and void material placed in the forms at the residence
under construction at Lot 72, Shenandoah, (202 Cheops), a subdivision of Fort Collins, Colorado.
This observation was performed on June 9, 1999.
It is our opinion that the reinforcement was in general conformance with MNA, Inc. foundation plan,
Job No. 1024.
