Subject: WORK AUTHORIZATION
To Whom It May Concern:
INVEX LTD. has been asked by Cermak. Peterka Petersen, Inc, for
authorization to insulate the south wall of Unit 4 located at 1415 Blue
Spruce Drive. They will be applying 2" "Thermax" insulation to the
interior of the wall, 20' in, height by 120' in length. It is applied
using construction adhesive.
By this letter, .we are authorizing Cermak Peterka Petersen, Inc, to
proceed with work.
