1. I am/we are acting on my/our behalf for the purpose of obtaining a building permit for the construction
or alteraltion of my/oulr home. The home to be constructed or altered is on the above property and
is mytour personal residence.
2. 1 am/we are personallyll performing all of the work, or supervising unpaid volunteers or "employees" (see
PPM 58 attached) who might be assisting with any of the work related to the construction of the above -
referenced home, which is described more fully on Permit Application No.(obtain from
Building & Zoning Office).
3, I/we have not personally constructed any other new home within the past 12-month period.
4. I/we understand that any persons or agents contracted to perform structural framing, plumbing, HVAC,
electrical work, or roofing, MUST BE licensed contractors in accordance with the regulations of Fort
Collins.

5. Vvve understand that failure to comply with any of the above conditions may result in revocation of any
permits associated with the above Application Number, and forfeiture of any fees that have been
collected.

EFFECTIVE DATE: Immediately
REQM,EMENTS FOR CONTRACTOR -EXEMPT STATUS
Owners of an existing or proposed new single-family home in Fort Collins may
PERSONALLY perform any construction of, or alterations to their new or existing
homes and associated accessory building(s) without any licensed contractors being
involved subject to the following conditions:
• The home must be the owner's personal residence and be classified by the
building code as an "R-3 Occupancy" (a single-family home on an individually
deeded lot).
• Un-paid volunteers must be supervised directly by the owner.
• Any other workers must qualify by local and state regulations as "employees",
be supervised directly by the home owner -builder, and be personally paid an
hourly wage or salary -- not paid through a contractor paid through a company
or firm name. Any paid worker who is not an employee of the home owner -
builder and is acting as an "independent contractor" as defined by state law
MUST BE licensed by the City as a contractor.
• All paid subcontractors performing structural framing, plumbing, HVAC,
electrical installations, or roofing MUST BE licensed as contractors by the City.
• No more than one new home may be constructed.by the same owner(s) within
a 12-month period.
• An owner:builder must submit a notarized City "Home Owner -Builder

Affidavit" authorization form to thp'Buildii &.Zonincy Department before a
building permit can be issued.
Failure to comply with the above conditions:i:aa=result in the City issuing a "Stop
Work" order for the project, revocation of any permits,issued and forfeiture of fees
collected.
