'The plans and specifications submitted for the construction of Martin's Martini Bar, 234 Linden Street,
Fort Collins, Colorado, have been reviewed by this department. The following items were not clearly specified
in the plans or were not in compliance with the Rules and Regulations Governing the Sanitation of Food Service
Establishments in the State of Colorado. These items must be clearly specified in writing or be modified to
be in compliance with the above regulations prior to approval of the submitted plans. The plans are approved
at this time. All plans must be approved prior to commencement of construction.
1) Approved.
3) Be advised that no appurtenances, such as syrup lines, carbonators, drop -in cold plates, drain lines
or bottle holders are permitted in potable ice bins or ice machines or any unit used to store potable
ice.
9) Be'advised, drainboards must be self draining and at least 36" in length when multi -use dishes are used.
10) Provide mop hangers or hooks around the mop sink to hang and air dry mops.

inspection and thereafter:
a) stab food thermometer (0 to 220 F)
b) food grade plastic gloves and/or finger cots
c) approved sanitizer
d) test strips for the sanitizes
The written plans submitted are approved at this time. Be advised approval of plans;, including the
___!!tabovdiniodincations, does -riot constitute accepiance`of the'completea structure. An -ins ctidd-of-the-premises-
must ,be conducted by this department prior to approval for operation and licensure of the establishment.
Failure to follow the recommended changes may prevent or delay the licensure of this establishment. If
construction is not commenced within 120 days, any approval of the plans will be null and -void and plans will
have to be resubmitted along with a $75.00 plan review fee for a second review.
Please notify the department at least 48 hours prior to your opening date so' that a final acceptance
inspection can be performed.
This department's written approval and the receipt of a completed state food service license application
and applicable fees is necessary prior to opening.

The plans and specifications submitted for the construction of Martin's Martini Bar, 234 Linden Street;
Fort Collins, Colorado, have been reviewed by this department: The following items were not clearly specified
iri the plans or were not in compliance with the Rules and Regulations Governing the Sanitation of -Food Service
Establishments in the State of Colorado. These items must be clearly specified in writing or be modified to
be in compliance with;the above regulations, prior to approval of -the submitted, plans. The plans are not
approved at this time. All plans must be approved prior to commencement of construction:
1) Some of the information submitted with the revised plans appears. to be in conflicealready submitted
and approved information. Please address the following items:
A) "According to the resubmitted plans Item #5 is actually Item.#4 (handsinks). 'This fact confuses the
equipment layoutsinthe floor plans for the kitchen and bar..: -
b) The original 4-compartment sink was specified witty bins of 14x10x10 cu. inch volume. Paragraph
#8 -of the resubmitted plans indicates that the bin' volumes.are '12x12x18 cu. inches..
c) The revised -plans "show await station on the west end of the liar on the dining area°side of the bar
counter: No information has been rovided'about this area
Please submit.revised floor plans for the kitchen, bar; and wait station, showing all equipment,
numbered, and accurately corresponding to alike numbered equipment listing. Include make and model
-.numbers for all equipment. Include information on what the wait station will be used for.
2) Acknowledged.
3) Beadvised-that no appurtenances; such as syrup lines,' carbonators, drop -in cold plates, drain lines

recirculate air.

14) The resubmitted plans indicate that the floor sink for walk-in compressor drainage will be located inside
the walk-in itself. The floor sink -for the walk-in must be located outside of the walk-in. Please
provide another location for this drain.. Be advised, floor sinks must not be installed under
equipment or cabinetry where they cannot be cleaned. All floor sinks and floor drains must be
installed so that they are flush with the finished floor.

21) Be advised,'30 foot candles of light are required on all working surfaces and 20 foot-candles of light
on all other surfaces and' equipment in the kitchen, storage areas, service counter, waitress station,
restrooms, bar, utensil washing area, janitor's closet, and all food prep areas.' At least 10 foot candles
of light must be provided in all areas of the walk-in cooler and freezer. All lights in the above areas,
_ excluding the restrooms and janitors:closet,.must be shielde&in an approved manner. Be advised, all
lights inside equipment must. al -so —lie- "shielded. `Provide a `completeaiglifing`scliedalF ci the bai,
kitchen, restroom, supply/liquor storage, -and walk-in areas.
22) In paragraph #22 and exhibits #22 of -the resubmitted plans a "bar top" and a "bar cover" are referred
to. Please indicate if these are the same item or two separate items, and where each one is located.
Be advised, floors, walls, ceilings, counter tops, storage shelves, cabinets and -all work surfaces located
within the kitchen, storage rooms, janitor's closet, restrooms, waitress station, serving area, bar,
dishwashing areas, walk-ins, and all food prep.areas must be of an approved material that is smooth,
easily cleanable, nonabsorbent and of a light color.

The plans are not approved at this time. Once items # 1, 14, 21, and 22'have been submitted in
writing, the plans will be reviewed for final approval. After the last item is resubmitted, the department will
review the plans and resubmittals within 10 days.
Be advised that Colorado Revised Statutes (1989) 12-44-206 require plans and specifications be
submitted to the local health department and approved prior to beginning construction.
