TO PLAINTIFF:

for the following defendant: Logan Simpson. If plaintiff has obtained service, plaintiff must file
proof of service. If service has not been obtained, plaintiff should advise the court in writing as
to the status of the case and when service is expected to be had on defendant.
If service on defendant is not made within 63 days of the complaint being filed, the court
shall dismiss the above-named defendant without prejudice unless good cause is shown to extend
the deadline. C.R.C.P. 4(m).
