SUBMITTAL REQUIREMENTS:
1) The initial submittal and additional rounds of review shall be electronic (one (1) copy on a CD or Flash drive) of
the site, landscape, elevation, etc. - whichever sheets are being altered. A Legal Description is required. Changes
on each sheet should be clouded.
Once all departments approve the proposed changes a Final Hard Copy will be required to be printed on 24"06"
Mylar sheets. Please ensure the electronic submittal can be printed to scale on 24"06" sheets.
2) Complete and sign Minor Amendment application form and Transportation Development Review Fee application
form (TDRF). The following link is to the TDRF application form:

3) Application fee of$350. City has agreed to waive - see attached email correspondence
Note: an additional $250 fee is required if the Poudre Fire Authority (PFA) needs to review the application.
Typically, PFA fee is required for all changes of use, building additions and/or new buildings.
MINOR AMENDMENT
Detailed description of the change and reason(s) for the request:
Tree caused damage to driveway and foundation and had to be removed. Landscaping was updated with
approval from the HOA. Neighbors took it upon themselves to file a complaint with the City because they prefer
to live in a homogeneous neighborhood where everything looks the same. Homeowners love planet earth and
want to save water by using drought tolerant plants.

I certify the information and exhibits submitted are true and correct to the best of my knowledge and that in filing this application, I am acting with
the knowledge, consent, and authority of the owners of the property (including all o%cners having a legal or equitable interest in the real property, as
defined in Section 1-2 of the City Code; which is the subject of this application) without whose consent and authority the requested action should not
lawfully be accomplished. Pursuant to said authority, I hereby permit City officials to enter upon the property for the purpose of inspection, and if
necessary, for posting a public notice on the property.

MEDIATED AGREEMENT
Case Number: 1007
On the 241h day of April, 2018, parties, Lynne Pearson, Judy Dunn, & Randy Lucas, voluntarily met at the
City of Fort Collins' Mediation & Restorative Justice Services Office to discuss their issues and explore
potential solutions. They have mutually agreed to the following terms and conditions:
1.) Randy Lucas will replace the pine tree, move the Yucca, remove Cholla closest to the driveway,
�� remove the dead small spruce tree. These items will happen prior to June 1, 2018.
2. Mandy Lucas will complete a landscape plan and submit it to the Cobblestone Corners HOA Board as
,,V- an Architectural Change Request Form. This will include:
.Streambed_of mixed river cobble (2 to 2.5 feet wide)
Add color plants (perennials, pollinators) approximately 15 small plants
c The smaller cactus are fine and will not be remove
In the future, Randy Lucas, Judy Dunn, and Lynne Pearson will discuss landscape plans before taking it
to the HOA board. This discussion can take place before May 20, 2018
The other plants that are currently planted are acceptable and will not be moved
Furthermore, the parties agree:
• This Agreement is a binding agreement; while this Agreement is not a court order and the City of Fort
Collins Mediation Program will not enforce this Agreement, Colorado Courts have upheld written
mediation agreements as binding. The parties are responsible for understanding and inquiring about
the impacts and consequences of this Agreement on other legal agreements, covenants, court decrees,
rules, and orders to which the respective party is or may be bound, and that the mediator, the City of
Fort Collins Mediation Program, and the City of Fort Collins makes no representations about any such
impacts.
• They have been previously advised that if they desired legal advice, they needed to obtain their own
legal counsel.
• All mediation communications are confidential under Colorado law. However, this Agreement is not
confidential unless all parties have agreed it should be and unless confidentiality is listed above as a
term and condition of this Agreement.
• The terms and conditions of this Agreement will be adhered to from this date forward. If any terms of
the Agreement become inapplicable in the future, the parties may either rework the Agreement
themselves or request another mediation session.
