As requested, during the period of July 15 through July 27, 2019, Earth Engineering Consultants,
LLC (EEC) personnel provided construction observation and testing services for the referenced
project in Fort Collins, Colorado. The services provided during the referenced period were on a
part-time/on-call basis and included observations and field density testing of curb and gutter,
roadway, and sidewalk subgrade materials, roadway aggregate base course (ABC) and hot mix
asphalt (HMA) pavement materials and testing of cast-in-place concrete as described herein for
the referenced project. The results of the observation and testing services completed by EEC
personnel during the referenced period are included with this report.
Observations and Field Density Testing Services
On July 24, 2019, a proof roll observation was conducted on the prepared pavement subgrades
for the interior roadways and along the widening portion of West Elizabeth Street. The proof
roll consisted of visual observations of the effects with a loaded dual axle water truck
maneuvering along the roadway alignments. At the time of observation, the subgrades appeared
to be stable/suitable for placement of the pavement section.
During the referenced period, EEC personnel performed field density tests on prepared subgrade
materials within curb, gutter and sidewalk alignments as well as of ABC pavement materials as
presented herein, that had been placed and compacted prior to and/or during our various site
visits. The field density tests were completed at random locations within the areas tested and
were performed in general accordance with ASTM Specification D6938. Results of the field
density tests completed during the referenced period are shown on the attached summary sheet

and approximate locations are “plotted” on the attached site diagram included with this report.
The field density test results met the compaction requirement of at least 95% of standard Proctor
maximum dry density and were within ±2% and/or ±3% of the material’s optimum moisture
content, where applicable.
Laboratory and Field Density Testing of HMA Pavement Material
During the referenced period, EEC personnel observed paving operations and obtained a sample
of the Grading S(75) HMA pavement materials, (Kehn Construction – Mix ID 638119) from the
site during paving operations along West Elizabeth Street as identified herein. Laboratory
testing for the collected HMA sample included determination of the binder content (by means of
ASTM D6307) and washed sieve analysis of the extracted aggregates (ASTM Specifications
C117 and C136). The percent asphaltic cement (AC) content and extracted aggregate gradations,
as presented herein, were performed without an ignition oven calibration of the aggregate and
AC materials used to produce the Grading S HMA materials. The uncorrected HMA laboratory
test results were within the acceptable oil content range for the approved mixes (i.e., within +/-
0.3% of optimum AC content). For the extracted aggregate portion, the results were, generally,
within the approved job mix formula tolerances for the Grading S mix. The results of the
laboratory tests completed by EEC personnel are indicated on the attached summary sheets.
After compaction of the HMA pavement materials, EEC personnel performed density tests on
the in-place pavement using a nuclear moisture-density gauge. The density tests were completed
at random locations as shown on the enclosed summary sheets within the pavement improvement
areas, and were performed in general accordance with ASTM Specification D2950. Results of
the field density tests completed during the referenced period are indicated on the attached
summary sheet. Those test results were within the project compaction requirements of 92% to
96% of the material’s maximum theoretical specific gravity (Rice Value) as indicated on the mix
design as provided to us by Kehn Construction.
Testing of Cast-in-Place Concrete
During the referenced period, EEC personnel observed, and tested Portland cement concrete
delivered to the site for placement as indicated on the enclosed daily reports for the referenced
project, (please refer to Set Nos.2 through 4). Field testing performed on the plastic concrete
