Project Description: This is a request for a Major Amendment for a change in use to utilize an
existing building for an adult day/respite care; Elderhaus. A plat is required
because the property has never been platted before. The project is located at
6813 S College Ave and it is in the M-M-N (Medium Density Mixed -Use
Neighborhood District) zone district.

Issue: 12/29/2015: The applicant had asked if the 5 foot easement around the property that was requested by
Comcast was needed. According to Comcast - "as long as Comcast has a Utility easement on the north
end of the property/ driveway to get to this property then Comcast will not need a 5 foot easement around
the whole property."

Issue: 12/29/2015: Here is what is needed to get the access and emergency access deed that is being dedicated
to the City finalized and recorded.
• 1 need a check submitted for $250 made out to City of Fort Collins. This is for the City's processing fee
- to cover the cost of reviewing the document and processing it.
• 1 need the fully signed document submitted to me (all signatures but the City's). And then I can route it
to obtain Rick Richter and the City Clerks signatures and get it filed.
• 1 also need a check or cash for the filing fees. Based on the document being 7 pages long the filing
fees will be $41. This check needs to be made out to Larimer County Clerk and Recorder.
Once we have the final document and can get it fully signed and recorded I will let you know what the
recording number is so that we can get that inserted and noted on the Plat.
