Timeline • Catholic Charities will end their role as operator of NCS on November 3 to focus on the overflow shelter at their site for women and families. • Homeward Alliance will begin serving as case management for NCS clients on November 4. • The end date for the transition program using current funding stream is December 15, 2020. • To ensure safe transition for highest-risk clients, NCS may extend into 2021 using CDBG-CV funds. Due to federal funding restrictions, the program would run no more than 100 days into 2021. Status • As all participants currently being served at NCS have been prioritized due to health risk factors related to COVID-19, Homeward Alliance will continue to serve these participants with the aim of securing housing. • Final intakes for NCS were completed on October 23, and no new participants will be added to the program. • There are currently 38 participants at NCS, and two people have been exited into permanent housing. • Several Homeward Alliance staff positions have been filled and assigned to this project. • Most participants are getting document ready for housing so they can move quickly once housing is identified. Funding • Homeward Alliance will cover personnel costs, including a full-time resource navigator and full-time case manager. • The City of Fort Collins will continue to use CARES Act-CVRF funding to cover hotel, security, and food costs, estimated at $150,000 for November and December 2020. • Any continuation of NCS into 2021 will use CDBG-CV funds. Communication • Case managers at the NCS site are in the process of communicating the service provider transition to participants in the program. SummitStone Health Partners and the Health District of Northern Larimer County will continue to support Homeward Alliance in case management and resource navigation. Participant Stories & Feedback: • The NCS Program Manager shared a few updates: o “One client… has been stabilized enough to not only continue to be a dependable employee despite being an amputee, but she also has an opportunity to be the mother her son needs her to be right now.” o “We have… an amazing woman… who is caring for her 18-month-old great grandson full-time… This dedicated great grandmother is in a safe and stable environment.” o “A partially blind 69-year-old woman is homeless for the first time having recently cut ties with her abusive family… During her time here, she has ordered a new ID, applied for Social Security Benefits, and is working on securing Medicare with the help of the resource navigators.” • One NCS participant passed away at the hospital due to complications of a chronic medical condition. Rose, 45, was “loved and supported by an incredible and diverse team of competent, passionate, and dedicated individuals during this final chapter… She did not pass away alone, unsupported or unforgotten.” Shared by Catholic Charities. • One woman, with stage 4 cancer, wrote, “This is exactly where I need to be… it really was life or death.
