SUBJECT
First Reading of Ordinance No. 001, 2013, Appropriating Unanticipated Grant Revenue in the Recreation Fund for the
Vida Sana Program.
EXECUTIVE SUMMARY
The Recreation Division was awarded a seven month sub-grant from the Coalition for Activity and Nutrition to Defeat
Obesity (CANDO) in the amount of $54,052. The Recreation Division will utilize these funds to hire a part-time
program coordinator, two bilingual program staff and to fund program participant admissions to the Northside Atzlan
Community Center.
BACKGROUND / DISCUSSION
The sub-grant from CANDO will fund salaries to support a program coordinator, two program staff and exercise
coaches who will be responsible for organizing and facilitating bilingual physical activity programming targeted for
Latino/Hispanic community members at risk for chronic diseases. Additionally, funds will support supplies, operating
costs and free passes for admission to NACC by participants. The time frame for this program is December 11, 2012
– June 30, 2013.
FINANCIAL / ECONOMIC IMPACTS
$54,052 will be available to support the program. $27,590 will be used for salaries for the Program Coordinator and
two bilingual program/exercise coaches. $26,462 will be available for operating costs, purchase supplies, and to
reimburse NACC for free pass admissions for participants.
STAFF RECOMMENDATION
Staff recommends adoption of the Ordinance on First Reading.
PUBLIC OUTREACH
Three block parties were held at Coachlight, Hickory and Poudre Valley Mobil Home Parks during the summer of 2012
to determine elements of citizen concerns. Surveys were completed to help determine what these communities want
in the way of programs and services.

Memorandum of Understanding
This Memorandum of Understanding (“MOU”) is effective as of October 1, 2012 (Effective Date) by and
between Poudre Valley Health System Foundation (“PVHSF”) and The City of Fort Collins, a Colorado
municipal corporation (“City”), for the benefit of the City’s Recreation Department.
CanDo is a program sponsored by PVHSF. PVHSF is the fiscal agent who administers all grants for CanDo.
CanDo has received a grant from the Colorado Department of Health and Environment’s (CDPHE) Office of
Health Disparities to fund the Vida Sana project for a nine month contract beginning October 1, 2012 and
commencing June 30, 2013. This MOU sets out the terms and conditions of how the grant money will be used
by the City. See Appendix A for detailed budget information and Appendix B for detailed grant implementation
responsibilities.
The City and PVHSF agree to the following:
Grant Request and Distribution of Funds:
1. PVHSF, through CanDo, has been awarded $54,052 from CDPHE for culturally congruent exercise
programming through Northside Aztlan Community Center (“NACC”) as part of the Vida Sana Grant.
2. PVHSF, through CanDo, agrees to disburse the amount approved by CDPHE in monthly reimbursable
payments.
a. Disbursement of funds from PVHSF to the City of Fort Collins will be made as follows:
$27,590 for salaries to support program coordinator, 2 program staff and exercise coaches
$26,462 in supplies, operating costs and free passes for admission to NACC by
participants.
b. The City will submit invoices for the month’s prior expenditures to the PVHS Healthy Equity
Coordinator by the 15th of each month. The Health Equity Coordinator will review all invoices
and submit them to the PVHSF accounting staff for payment.
Responsibilities of the City:
The City, through the NACC, will provide the access to culturally relevant physical activity component to this
project. NACC staff will work with community members served by the promotoras (health access resources for
the neighborhoods identified in this grant, funded from a separate source) and others to expand existing
programs and create new opportunities based on participant feedback. NACC will track members’ participation,
progress and outcomes of the increased access to physical activity opportunities. Moreover, NACC, as a trusted
and reputable location for members of the target population, will serve as a site for additional program/classes
generated by promotoras and their neighborhoods and will lead efforts to add in-neighborhood programs for
years two and three of this proposal. NACC staff will also assist with connecting Vida Sana participants to other
facets of community efforts currently underway to address health equity such as the ACHIEVE project, Bike
and Pedestrian Education Coalition, etc.
Position Responsibilities:
1) 0.5FTE Bilingual Program Coordinator will be responsible for coordinating NACC’s involvement in
this project including, but not limited to,

a. Meeting regularly with PVHS’s Health Equity Coordinator and Women’s Resource Center staff
to collaborate about the implementation of the Vida Sana project
b. Attending monthly Vida Sana coalition meetings and engaging members in the project
c. Meeting with promotoras, families, and current class participants to assess desires for new
programs and develop and implement them
d. Develop and maintain a tracking tool to measure all required elements of physical activity
among participants outlined in this project.
e. Overseeing the program staff and exercise coaches and meeting regularly to ensure the program
is being implemented comprehensively
f. Successfully market new and existing exercise programs offered at NACC through the Vida
Sana program
g. Develop an implementation and evaluation plan for the distribution of the 20,000 free passes
h. Assist with reporting requirements and evaluation of program
i. Ensure invoices/accounts payables are submitted by the deadline noted above to PVHSF
2) Program staff (9 hours/week) will:
a. Work with program coordinator to facilitate new and existing programs at NACC for youth and
adults
b. Assist with tracking participants’ attendance, progress, etc.
c. Meet with promotoras and program coordinator to troubleshoot and develop new program
concepts based on participant feedback
d. Provide necessary information to program coordinator for reports, evaluation, etc.
3) Exercise coaches will:
a. Provide bilingual exercise consulting to clients referred through medical providers, promotoras
or other partner agencies. Exercise consulting must be based on American College of Sports
Medicine guidelines.
b. Implement a tracking tool to monitor clients’ progress in attaining physical activity goals
Hiring and retention of employees for the above positions will be in compliance with City policies and
procedures and all state and federal laws.
Termination:
1. In the event that due to factors outside of PVHSF’s control, funding is not available from CDPHE under
which CanDo and PVHSF are funding the subcontracted amount, PVHSF will notify the City
immediately and this MOU will terminate.
2. In the event that either party materially breaches this MOU and fails to remedy such breach after written
notice and a reasonable opportunity to cure the breach, then the non-breaching party may terminate this
MOU by providing written notice of such termination to the breaching party.

ORDINANCE NO. 001, 2013
OF THE COUNCIL OF THE CITY OF FORT COLLINS
APPROPRIATING UNANTICIPATED GRANT REVENUE IN
THE RECREATION FUND FOR THE VIDA SANA PROGRAM
WHEREAS, the Recreation Division was awarded a sub-grant from Poudre Valley Health
System Foundation and the Coalition for Activity and Nutrition to Defeat Obesity (CANDO) in the
amount of $54,052 for the Vida Sana program to eliminate racial and ethnic disparities among
Latino and Hispanic community members; and
WHEREAS, these funds were awarded to the Northside Aztlan Community Center to fund
culturally congruent physical activity opportunities targeted to Latinos and Hispanics; and
WHEREAS, the funds will be used to hire a part-time program coordinator and two bilingual
program staff, and to reimburse the City for free pass admissions to the Northside Aztlan
Community Center; and
WHEREAS, Article V, Section 9, of the City Charter permits the City Council to make
supplemental appropriations by ordinance at any time during the fiscal year, provided that the total
amount of such supplemental appropriations, in combination with all previous appropriations for
that fiscal year, does not exceed the current estimate of actual and anticipated revenues to be
received during the fiscal year; and
WHEREAS, City staff has determined that the appropriation of the revenue as described
herein will not cause the total amount appropriated in the Recreation Fund to exceed the current
estimate of actual and anticipated revenues to be received in that fund during any fiscal year.
NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE CITY OF FORT
COLLINS that there is hereby appropriated for expenditure from unanticipated revenue in the
Recreation Fund the sum of FIFTY-FOUR THOUSAND FIFTY-TWO DOLLARS ($54,052) for
the Northside Aztlan Community Center to fund physical activity classes and opportunities targeted
to Latinos and Hispanics.
Introduced, considered favorably on first reading, and ordered published this 15th day of
January, A.D. 2013, and to be presented for final passage on the 5th day of February, A.D. 2013.
