A regular meeting of the Liquor Licensing Authority was held March 1, 2018 in Municipal Court, 215
North Mason. Judge Kathleen M. Lane presiding.

Judge Lane approved the 2017 Annual Report.

Irbine Enterprises, LLC, dba
Justine's Pizza Fort Collins,
140 East Boardwalk Drive, Unit J, Stipulation Agreement Approved
Sara and Dave Irby, representatives for Irbine Enterprises, LLC, stated they have had a chance to
review the stipulation agreement.
Assistant City Attorney Scurlock summarized the stipulation agreement. She stated the Licensee has
agreed to a 15 day suspension, 5 days served ($200 fine paid in lieu of suspension), with 10 days held
in abeyance for 1 year for violating Colorado Liquor Code Section 12-47-901(1)(a.5) prohibiting the

service of an alcohol beverage to a person under the age of twenty-one years. The City received
payment at the hearing.
Judge Lane approved the findings, conclusion and order accepting the stipulation agreement as
presented for Irbine Enterprises, LLC, dba Justine's Pizza Fort Collins, 140 East Boardwalk Drive,
Unit J.

Joel Nordby, general manager, stated the employee was immediately terminated. He stated he has had
an opportunity to review the stipulation agreement.
Assistant City Attorney Scurlock summarized the stipulation agreement. She stated the Licensee has
agreed to a 14 day suspension, 4 days served ($200 fine paid in lieu of suspension), with 10 days held
in abeyance for 1 year for violating Colorado Liquor Code Section 12-47-901(1)(a.5) prohibiting the
service of an alcohol beverage to a person under the age of twenty-one years. The City received
payment at the hearing.

Assistant City Attorney Scurlock summarized the stipulation agreement. She stated the Licensee has
agreed to a 14 day suspension, 4 days served ($3481.51 fine paid in lieu of suspension), with 10 days
held in abeyance for 1 year for violating Colorado Liquor Code Section 12-47-901(1)(a.5) prohibiting
the service of an alcohol beverage to a person under the age of twenty-one years. The City received
payment at the hearing.

Boyd Hoback, regional manager, stated he has had an opportunity to review the stipulation agreement.
Assistant City Attorney Scurlock summarized the stipulation agreement. She stated the Licensee has
agreed to a 14 day suspension, 5 days served ($1214.23 fine paid in lieu of suspension), with 9 days
held in abeyance for 1 year for violating Colorado Liquor Code Section 12-47-901 (1) (a.5) prohibiting
the service of an alcohol beverage to a person under the age of oventy-one years. The City received
payment at the hearing.

Manager Registration of Mark O'Prey from

Beau Jo's Pizza, 205 North College, Approved
Mark O'Prey, manager applicant, stated he has been the assistant manager for several years and just
recently became the general manager. He briefly described the training required of all employees that
are involved with the sale and service of alcohol.
Judge Lane approved the manager registration of Mark O'Prey from BJ Management, Inc., of Beau
Jo's Pizza, 205 North College Avenue.

Transfer of Ownership of Bann Thai Restaurant,
2722 Council Tree Avenue, Suite #137, from

Kultida Charamnuay, representative for Prosperous Thai Restaurant, LLC, stated there have been no
changes to the physical premises and that there have been no changes to the days and hours of
operation. She stated every patron that orders an alcoholic beverage must provide valid identification
in order to be served. Ms. Charamnuay stated she has a current copy of the liquor code on premise.
She stated the previous owner has trained her.
Judge Lane approved the transfer of ownership of Bann Thai Restaurant, from Bann Thai Restaurant,

Denise Hakim, representative for Yum Yum Restaurant, Inc., stated she recently took over the
restaurant from the previous owner. She stated there will be no changes to the premises or the day to
day operations. Ms. Hakim stated any employee found in violation of the liquor code will be
immediately terminated.

Love Foods, LLC, to Yum Yum Restaurant, Inc.

FastnFriendly Enterprises, Inc., & 7-Eleven, Inc., to
7-Eleven, Inc., and Trade Name Change to 7-Eleven Store 35506j, Approved
Kevin Coates, attorney representing 7-Eleven, Inc., stated that this store will likely be transferred to a
combined franchisee and corporate store shortly. He stated that nothing will change with respect to
the operations of the store. Mr. Coates stated that any patron purchasing 3.2% beer is required to
provide valid identification.
Judge Lane approved the transfer of ownership of 7-Eleven Store 35506A, 3045 West Prospect Road,
from FastnFriendly Enterprises, Inc., & 7-Eleven, Inc., to 7-Eleven, Inc., and Trade Name Change to

Transfer of Ownership of Cozzola's Pizza,
241 Linden Street, from Mountain Dog, Inc., to
Haley Ventures, and Trade Name Change to Corina's Pizza, Approved
Corina Miller, representative for Haley Ventures, stated she has mostly new staff and that there are no
changes to the days and hours of operation. She stated any employee found in violation of the liquor
code will be immediately terminated.
Judge Lane approved the transfer of ownership of Cozzola's Pizza, 241 Linden Street, from Mountain
Dog, Inc., to Haley Ventures, and Trade Name Change to Corina's Pizza.

Transfer of Ownership of Pappy's Corner Pub, from
Double Play, LLC, to J and K Squared, Inc., and
Trade Name Change to Game Day Sports Grill,

James Quick, representative for J and K Squared, Inc., stated there has been no changes to the days
and hours of operation. He stated they have only made cosmetic changes and has mostly new staff.
Mr. Quick stated his daughter is currently running the establishment and has completed training. He
stated he will be moving to Fort Collins at the end of the spring semester and take over running the
restaurant. Mr. Quick stated anyone ordering an alcoholic beverage must provide valid identification.
He stated any employee found in violation of the liquor code will be immediately terminated.

and K Squared, Inc., and Trade Name Change to Game Day Sports Grill, 1027 West Horsetooth
Road, Suite 11.
Hearing on a New Application from Steamies Burger Bar, LLC dba
Steamies Burger Bar, 234 North College Avenue, Suite B2, Approved
Stanya Gorraiz, representative for Steamies Burger Bar, LLC, was sworn. She stated they are primarily
a restaurant and that they do not have a full bar service. Ms. Gorraiz stated she is currently TIPs
trained and that they likely won't open until June. In response to questions from the Authority, she
detailed the premises. She stated she will have approximately 6-8 employees and will always have a
manager on site. Ms. Gorraiz stated that any patron ordering an alcoholic beverage must provide valid
identification and will require all their employees to be TIPs trained prior to opening. She stated she
has not had a violation at their Telluride location.
Ellyn Just, representative for Aggie Agency, LLC, was sworn. She briefly described the petitioning
process and that this indicates support for this application.
Judge Lane adopted Resolution 2018 - 005 approving a new application from Steamies Burger Bar,
LLC, dba Steamies Burger Bar, for a hotel -restaurant liquor license at 234 North College Avenue,
Suite B2.
