A regular meeting of the Liquor Licensing Authority was held on November 19, 2012 in Municipal
Court, 215 North Mason. Judge Kathleen M. Lane presiding.

Approval of Minutes
Judge Lane approved the minutes from October 29, 2012.
Registration of April Anne Medeiros as
Manager of Sanford's Grub and Pub,
1526 Oakridge Drive, Approved
April Medeiros, manager applicant, briefly described her background. She stated Sanford's is a member
of Team Fort Collins and that she requires all her front -of -house staff attend training in the sale and
service of alcohol. Ms. Medeiros stated she supervises approximately 33 employees and that 16 are
directly involved in the sale and service of alcohol. She stated any patron ordering an alcoholic beverage
is required to present a valid id otherwise they will not be served a drink. Ms. Medeiros stated any
violation of the liquor code is immediately terminated. She stated she has a current copy of the liquor
code on premise.
Judge Lane approved the registration of April Anne Medeiros as manager of Sanford's Grub and Pub,
1526 Oakridge Drive.
Registration of Dane Bradford as
Manager of Chipotle Mexican Grill,
2120 East Harmony Road, Suite 107, Approved
Dane Bradford, manager applicant, stated Chipotle sponsors monthly training and that he has recently
attended this training. He stated the cashier is the employee responsible for the sale and service of
alcohol. Mr. Bradford stated any patron ordering an alcoholic beverage is required to present valid
identification. He stated their liquor sales area very small percentage of their gross sales. Mr. Bradford
stated any employee found in violation of the liquor code is immediately terminated.
Judge Lane approved the registration of Dane Bradford as manager of Chipodc Mexican Grill, 2120
East Harmony Road, Suite 107.

Transfer of Ownership of The Hatrixx,
6013 South College Avenue from Hatrixx, Inc., to
Home Run, LLC and Trade Name Change to Motherlode Saloon, Approved
Dave Wentz, Cara Strawn, and Terry Bell, all representatives for Home Run, LLC, were present. Mr.
Wentz stated that his employees at West End Pub are subject to random compliance checks put on by
the Responsible Alcohol Retailers. He stated there are no changes to the premises and that they are
working to upgrade the food quality. Mr. Wentz stated they have increased their food sales at Pappy's
Corner Pub by 18%. He stated he will require the employees from all three of their locations attend
TIP's training on a regular basis. Mr. Wentz stated when a patron orders an alcoholic beverage is
required to present valid identification. He stated he has a current copy of the liquor code on premise.
Judge Lane approved the transfer of ownership of The Hatrixx, 6013 South College Avenue from
Hatrixx, Inc., to Home Run, LLC, and trade name change to Motherlode Saloon.
Transfer of Ownership of Applebee's Neighborhood Grill & Bar,
4306 South College Avenue from Amrest, LLC to
Apple Colorado, LLC, Approved
Dill and Dill, Kevin Coates, attorney representative for Apple Colorado, LLC, stated nothing has
changed from the previous owner, other than the ownership. Mr. Kirby, registered manager, stated he
has been in the restaurant business for over 18 years. He stated there is always at least one manager on
premise. Mr. Kirby stated all employees must attend training to properly identify underage and/or
visibly intoxicated patrons. He stated any patron coming into the restaurant would not see any changes
to the menu or the operations of the restaurant.
Judge Lane approved the transfer of ownership of Applebee's Neighborhood Grill & Bar, 4306 South
College Avenue from Arrest, LLC to Apple Colorado, LLC.
Lounge Unspa, LLC,
dba She She Nail & Wax Lounge,
2601 South Lemay Avenue, #26, Approved
Susan Byrum, representative for Lounge Unspa, LLC, was sworn. She stated she and all her staff have
attended TIP's training. Ms. Byrum stated any patron ordering an alcoholic beverage is required to
present valid identification and that they have a two drink maximum per patron. She stated they are
members of the Responsible Alcohol Retailers organization. Ms. Byrum stated alcohol is stored in a
locked refrigerator. She stated alcohol sales are a small percentage of her gross sales. Mr. Byrum stated
any employee found in violation of the liquor code is immediately terminated. She stated she has a
current copy of the liquor code on premise.
David Wyatt, representative for Aggie Agency, LLC, was sworn. Mr. Wyatt stated there were several
people in the neighborhood could have signed the petition that were against this application, but refused
to sign in opposition. He stated overall, the signatures show support for this application.

Judge Lane adopted Resolution 2012-17 approving an application from Lounge Unspa, LLC, for a beer
and wine license at She She Nail & Wax Lounge, 2601 South Lemay Avenue, #26.
