A regular meeting of the Liquor Licensing Authority was held on June 6, 2011 in Municipal Court, 215
North Mason. Judge Kathleen M. Lane presiding.

Agenda Review
Deputy City Clerk Jensen stated that there were no changes to the agenda.
Registration of Adam Grove as
Manager of Chipotle Mexican Grill,
2120 East Harmony Road, Suite 107, Approved
Adam Grove, manager applicant, briefly described his background. He stated he has received extensive
training with regard to the sale and service of alcohol. Mr. Grove stated he supervises approximately
20 employees and that he has two service managers that is responsible while he is not on premise. He
stated he is responsible for training all employees. Mr. Grove stated he does not have a current copy
of the liquor code on premise. He stated any employee found in violation of the liquor code will likely
be suspended on their first offense and that any subsequent violations would be terminated.
Judge Lane approved the registration of Adam Grove as the manager of Chipotle Mexican Grill, 2120
East Harmony Road, Suit e107.
Request from Grandboid Enterprises, LLC dba
Crazy Jacks' Saloon to Temporarily Change, Alter, or Modify the Licensed Premises at
6520 South College Avenue on June 18, 2011, Approved
Thomas Grandbois, representative for Grandbois Enterprises, LLC, stated this temporary modification
of premise is to accommodate a fund raising event for a non-profit organization. He stated this is a
similar event to what he had last year and that he had no problems with last years event. Mr. Grandbois
stated he is expecting the same number of participants and will have the same level of trained staff to
operate the beer garden.
Judge Lane approved the request from Grandbois Enterprises, LLC dba Crazy Jacks' Saloon to
TemporarilyChange, Alter, orModifythe Licensed Premises at 6520 south College Avenue on June 18,
2011.

Transfer of Ownership of Times Square Dueling Pianos,
2631 South College Avenue from
Times Square Dueling Piano Bar & Grill, LLC, to
Times Square Dueling Pianos, LLC, Approved
Brant Frazier, representative for Times Square Dueling Pianos, LLC, stated he primarilyhas a corporate
background and is focusing on putting proper procedures in place to insure all rules and regulations are
adhered to. He stated he has retained much of the previous staff. In response to a question from the
Authority, Mr. Frazier stated he will include the front patio as part of his licensed premises. He stated
he will submit an updated diagram to Deputy City Clerk Jensen., Mr. Frazier stated he will be
emphasizing food sales. He stated he will be on -site most of the time and has keyemployees that can
step in to a managerial role. Mr. Frazier stated all patrons ordering alcohol will be carded. He stated
any employee found in violation will likely be suspended, however, subsequent violations will be
immediately terminated. Mr. Frazier stated if an employee intentionallyviolates the liquor code will be
immediately terminated. He stated he has a current copy of the liquor code on premise.
Judge Lane approved the transfer of ownership of Times Square Dueling Pianos, 2631 South College
Avenue from Times Square Dueling Piano Bar & Grill, LLC, to Times Square Dueling Pianos, LLC

for a beer and wine license at Tap and Handle,
307 South College Avenue, Approved
Adam Stapen, attorney representing Wise Fly, LLC, stated he has 4 witnesses and was readyto proceed.
Joshua Guernsey, resident and business owner, was sworn. He stated he lives at 926 Akin Avenue. Mr.
Guernsey stated he owns property at 306 West Laurel Street. He stated he is over 21 and occasionally
consumes alcohol beverages. Mr. Guernsey stated he is in support of this liquor license application..
Donald James, property owner, 307 South College, was sworn. He stated he has owned the property
for 51 years. Mr. James stated he is over 21 years of age and on occasion consumes alcohol beverages.
He stated he is in support of this liquor license application.
Bryon Holloway, resident, resident at 219 West Myrtle Street, apartment A, was sworn. He stated he
is over 21 years of age and on occasion consumes alcohol beverages. Mr. Holloway stated he is in
support of this liquor license application.
Jeff Willis, representative for Wise Fly, LLC, stated he has been in the hospitalityindustryfor 20+ years.
He described his employment history. Mr. Willis stated he will have a wide variety of beers and wines.
He stated he will be emphasizing food service. In response to a question from Mr. Stapen, Mr. Willis
stated he will have a full kitchen facility. He briefly described the diagram submitted with the
application. Mr. Willis stated the beer garden (patio) will be fully enclosed and only accessible through
the building. He stated he has never had a liquor violation and that he will be responsible for training

his staff. Mr. Willis'stated they will card every patron that orders an alcohol beverage. He stated he has
recently passed a compliance check Mr. Willis stated any employee found in violation of the liquor
code will be immediatelytemiinated.
Max Scott, Oedipus, Inc., briefly described the petitioning process. He stated the results of this survey
show strong support for this application.
Judge Lane adopted Resolution 2011-14 approving an application from Wise Fly, LLC for a beer and
wine license at Tap and Handle, 307 South College Avenue.

for a hotel -restaurant liquor license at The 415,
415 South Mason Street, Approved
Andre Mouton, representative for Mason Street Five, LLC, was swom. He stated that all employees
will be TIP's trained. Mr. Mouton stated that patrons that have consumed to much alcohol is `red
tagged' in their computer and that no-one will be allowed to serve them He stated he will be acquiring
locally grown food to use in their restaurant. Mr. Mouton stated the emphasis for this restaurant will
be food. He stated they are still waiting for their permit and hope to be open by September. Mr.
Mouton stated he expects their alcohol sales to be approximately30-35% of gross sales. He stated they
will be open for breakfast, lunch and dinner. Mr. Mouton stated they will not be open past midnight.
In response to questions from the Authority, Mr. Mouton described the proposed licensed premise.
Justin Beamhardt described the petitioning process. He stated they have strong support for this
application from the neighborhood and that he received many positive comments.
Judge Lane adopted Resolution 2011-16 approving an application from Mason Street Five, LLQ for a
hotel -restaurant liquor license at The 415, 415 South Mason Street.
