A regular meeting of the Liquor Licensing Authoritywas held on July 18, 2011 in Municipal Court, 215
North Mason. Judge Kathleen M. Lane presiding.

Agenda Review
Deputy City Clerk Jensen stated that there are no changes to the agenda as presented.
Request from The Fogo Group, LLC, dba
The Rodizio Grill to Temporarily Change, Alter, or
Modify the Licensed Premises at 200 Jefferson Street for
the Brazil Fest for September 9, 10, and 11, 2011, Approved
Ryan Houdek, representative for The Fogo Group, LLC, briefly described the event he is requesting
the temporary modification of their liquor license. He stated he had no issues with this event last year.
Mr. Houdek stated he received manypositive responses in support of this application. In response to
a question from the Authority, Mr. Houdek stated the people he petitioned were not aware of this event
as it is so low key.
Judge Lane approved the request from The Fogo Group, LLC, dba The Rodizio Grill to temporarily
change, alter, or modify the licensed premises at 200 Jefferson Street for the Brazil Fest for September
9, 10, and 11, 2011.
Request from The Criterion Group, LLC dba
The Melting Pot to Temporarily Change, Alter, or
Modify the Licensed Premises at 334 East Mountain Avenue for
the NewWest Fest for August 12, 13 and 14, 2011, Approved
Ryan Houdek, representative for The Criterion Group, LLC, stated he petitioned for this event at the
same time as The Rodizio Grill and that he explained the difference between the two events. Liquor
Enforcement Officer Bendzsa requested that Mr. Houdek to keep the sidewalk open for emergency
personnel. He stated last year there were problems with keeping the sidewalks open at the event last
year. Mr. Houdek stated he was aware of problems last year and will work with emergency personnel.

Judge Lane approved the request from The Criterion Group, LLC dba The Melting Pot to temporarily
change, alter or modifythe licensed premises at 334 East Mountain Avenue for the NewWest Fest for
August 12, 13 and 14, 2011.
Registration of Christopher C. Hadfield as
Manager of Tavema Greek Restaurant,
4235 South College Avenue, Approved
Christopher C. Hartfield, manager applicant, stated he is the new manager. He stated he is TIP's
certified, receiving that certification within the last two months. Mr. Hartfield stated he is responsible
for training all the employees. He stated anyone who appears to be 30 years or younger is carded,
however, if they do not have an id, they will not be served. Mr. Hartfield stated any employee found
in violation of the liquor code is required to be retrained. He stated if it is an intentional act, they will
be immediately terminated. Mr. Hartfield stated he has a current copy of the liquor code on premise.
Judge Lane approved the registration of Christopher C. Hartfield as manager of Tavema Greek
Restaurant, 4235 South College Avenue.
Transfer of Ownership of Pappy's Corner Pub,
1027 West Hors etooth Road, Suite 111
from Tee Shots, LLC, to Double Play, LLC, Approved
David Wentz and Cara Strawn, representatives for Double Play, LLC, stated they will not be changing
much at this location. Mr. Wentz stated he is primarily a sports bar theme restaurant and will remain
that way. He stated he will develop a policy and procedures manual for Pappy's similar to what they
have at West End Pub. Mr. Wentz stated an employee found in violation of the liquor code which
could be deemed as an intentional act will be immediatelyternvnated. He will require all employees to
be retrained. Mr. Wentz stated he is a very hands on manager.
Judge Lane approved the transfer of ownership of Pappy's Comer Pub, 1027 West Horsetooth Road,
Suite 111 from Tee Shots, LLC, to Double Play, LLC.
Hearing on a New Application form 5 Brighams, Inc., for
a Hotel -Restaurant Liquor License at
d'Vine Bistro, 2580 East Harmony Road, # 100, Approved
Howard and April Brigham, representatives for 5 Brighams, Inc., was swom. Mr. Brigham stated he
has never had a liquor code violation and is ServeSafe certified. He stated he received this certification
November, 2010. Mr. Brigham detailed the concept of the restaurant. He stated they will offer an
extensive wine selection and limited beer and spirituous selections. In response to questions from the
Authority, Mr. Brigham described the premises. He stated they will be open from 10:00 a.m. to 10:00
p.m , seven days a week. Mr. Brigham stated they are currently serving lunch and that he anticipates
alcohol sales to be 30% of their gross sales. He stated any employee found in violation of the liquor
code will be immediatelytemunated.

Tina Scott, representative for Oedipus, Inc., was sworn. She briefly described the petitioning process.
Ms. Scott stated this establishment has a favorable reputation in the neighborhood. She stated she
received many favorable responses to this application.
Judge Lane adopted Resolution 2011-18 approving an application from 5 Brighams, Inc., for a hotel -
restaurant liquor license at dVine Bistro, 2580 East harmony Road, # 102.
